Refer a patient to Preferred Behavioral Health
Psychiatric medication management, counseling, and REMS-certified Spravato treatment for children and adults in the Metro East. We accept referrals from primary care, OB/GYN, emergency departments, therapists, schools, and community agencies.

What to send
Include with the referral
- Patient demographics: name, date of birth, phone
- Insurance: front and back of the card
- Reason for referral and the service requested (medication management, counseling, or Spravato)
- Current medications and recent clinical notes
- For Spravato: diagnosis and prior antidepressant trials (names, doses, durations)
Please don't send referrals by email. For urgent needs, call the office so we can help find the fastest option. For emergencies, direct the patient to 911 or 988.
What happens next
After you fax
- We contact the patientOur intake team calls the patient to verify insurance and schedule.
- We confirm with your officeYou'll hear back once the patient is scheduled, or if we can't reach them.
- We keep you updatedWith the patient's consent, we share treatment updates with the referring provider.
Spravato referrals
Who to refer for Spravato
- Adults with treatment-resistant depression, typically after two or more adequate antidepressant trials
- Adults with major depressive disorder with acute suicidal ideation or behavior, together with an oral antidepressant
- Patients able to stay about 2 hours after each dose and arrange a ride home
